Children & Family Wellness Center LLC, in partnership with Better Family Foundation, provides strength-based, trauma-informed, and family-centered services for youth and young adults involved with juvenile justice, probation, diversion programs, foster care, behavioral-health systems, school discipline, and other systems of care.
We recognize that justice-involved youth are more than their circumstances, court cases, or past decisions. Every young person deserves meaningful opportunities to heal, learn, accept responsibility, strengthen family relationships, develop positive skills, and build a successful future.

Our Services

We help youth prepare for and manage the transition home following juvenile hall, residential treatment, foster-care placement, behavioral-health treatment, or another out-of-home setting.
Support may include re-entry planning, family preparation, goal development, resource coordination, school reintegration, and connections to community-based services.

Each youth’s needs, strengths, challenges, goals, family relationships, educational status, and existing services are considered when developing an individualized support plan.
Plans may address:
Court or probation requirements
Education and school attendance
Family relationships
Emotional and behavioral wellness
Employment preparation
Independent-living skills
Community involvement
Personal goals and future planning

Our case-management services help youth and families identify needs, establish goals, access services, coordinate care, and monitor progress.
Case managers may collaborate, with appropriate authorization, with parents, caregivers, probation officers, attorneys, schools, behavioral-health providers, social workers, mentors, and other approved service partners.

We help youth and families understand service recommendations, diversion plans, probation expectations, and court-ordered requirements.
Support may include:
Compliance planning
Appointment and service coordination
Progress monitoring
Community-resource referrals
Community-service coordination
Documentation of program participation
Preparation for meetings with service-system partners

System involvement and out-of-home placement can affect trust, communication, family roles, and emotional safety. Our family-centered services help youth, parents, and caregivers rebuild relationships and develop healthier ways of communicating.

When available and approved, we help youth and families identify transportation options and community resources needed to access appointments, education, employment, court-related services, and other supportive programs.

Service Availability and Financial Assistance
Services may be available individually, in groups, virtually, in the community, or through coordinated agency partnerships.
Children & Family Wellness Center LLC provides private-pay and contracted services. Through our partnership, Better Family Foundation may offer reduced-cost or sponsored services to eligible youth and families based on program availability, funding, income, family size, financial hardship, and other eligibility requirements.
